With a view to taking care of the chronic patient and continuity of care, new technologies play an ever greater role in the development of user-centered integrated home services. In this perspective, the Sisifo Consortium proposes the telemedicine service dedicated to home care, to launch a qualitative transformation of the care / health world, in which care is complex and collaborative and the caring-cured role becomes crucial.

Telemedicine is the set of medical and IT techniques that allow the treatment of a patient at a distance. It favors the continuity of care and reduces its cost, improves patient self-management and clinical outcomes as a whole. Telemedicine is used successfully in many fields and finds particularly significant application with notable advantages in home health care.
